Student Enrolment

Application forms regarding student enrolments can be found below.

The following items must be provided with the application:-
- Birth Certificate
- Immunisation Records no more than 2 months old can be downloaded from your My Gov Account
- Proof of Residence in the form of a gas or electricity bill
-Details of family courts orders / parenting plans (if applicable)
- Details of any medical conditions, asthma, anaphylaxis, disabilities etc.

If both parents were born outside Australia, please provide one of the following documents:
- Australian Citizenship Certificate Or Australian Passport
- Visa / Permanent Residency

or shared living arrangements where none of the above can be produced, you must provide:
- A completed and signed Statutory Declaration, witnessed by authorised person
- A copy of the home owners gas or electricity bill
- A driver’s licence, bank statement or Centrelink letter in your name with corresponding address

For Pre-Primary to Year 6 Enrolments
- Copy of last school report
